Here's what the human RCT data actually shows. Historical articles may no longer reflect current regulatory status. Check the cited primary source.
NAD+ (nicotinamide adenine dinucleotide) has the deepest scientific literature of any compound in the PeptideX index — over 16,600 PubMed citations spanning seven decades of research. It is also one of the few longevity compounds with multiple completed randomized controlled trials in humans.
NAD+ is an endogenous coenzyme present in every cell. Its two critical functions in aging biology: (1) as a redox molecule in mitochondrial oxidative phosphorylation, essential for ATP production; and (2) as a substrate for sirtuins (SIRT1–7) and PARP enzymes that regulate DNA repair, gene expression, and cellular stress responses.

This decline correlates with reduced mitochondrial function, impaired DNA repair, and metabolic dysfunction.
Multiple Phase I/II trials using NAD+ precursors (NMN and NR) have demonstrated that oral supplementation reliably increases blood and tissue NAD+ levels in humans. A November 2025 eClinicalMedicine (Lancet) trial on nicotinamide riboside showed improvements in NAD+ levels with some cognitive and inflammatory markers improving in older adults. A 2024 long-term NR trial in ataxia telangiectasia patients showed improved neuromotor coordination over 2 years — the longest NR supplementation trial published.
The honest summary: NAD+ supplementation reliably raises NAD+ levels. Whether this reliably produces longevity outcomes in healthy humans remains an active research question.
NMN (beta-nicotinamide mononucleotide), a key NAD+ precursor, had its dietary supplement status challenged by the FDA in 2022. In December 2025, FDA reversed its position and confirmed NMN may be lawfully marketed as a dietary supplement — resolving a 3-year period of uncertainty.
Human RCT evidence exists for NAD+ precursors. Status not verified. No product-specific FDA primary source is recorded for this entry. FDA approval and compounding eligibility are not verified. Research information is not evidence of FDA approval. Approval, where applicable, is specific to a drug product, indication and route. Regulatory source not verified; no checked date is assigned. Score ceiling of 5 would require consistent human RCT outcomes on longevity endpoints specifically.
